We are here for you every step of the way. Here is what you can expect when filing a new workers’ compensation, disability, leave of absence or other type of claim with Sedgwick.

Let’s get started
There are four main stages in the life of a claim, and it all starts here. Click “start a claim” to get your initial instructions about any forms or paperwork you may need to complete.
If the claim is through your company or insurance company, an information packet may have been sent to you already via mail or email.

Time for forms
Fill out any forms or paperwork needed and send back to Sedgwick. Depending on your type of claim, some forms may need to be filled out by a medical provider as well.
Once everything has gone through, you should receive confirmation information so you can log into our 24/7/365 customer service portal.

Meet your adjuster
We will get in touch as soon as we know what types of benefits you qualify for, the amounts and when they begin.
This is also when you will have an adjuster assigned to you, who will be your primary contact and helpful guide throughout the rest of the process.

road to recovery
Your adjuster will be by your side as you take additional steps together for recovery, repairs and/or return to work. They can answer any questions you have, keep you updated on progress or provide you with any other information you need.
